This is where you can pay for something other than one of the standard membership products, for example a price adjustment to a previous order or a donation to club funds.

PLEASE DO NOT USE THIS FORM TO PAY FOR YOUR MEMBERSHIP !

 •  To join the club, please use the New Member Application form.
 •  To rejoin the club, please use the Rejoining Member Application form.
